In pediatrics ,we usually come across conditions where we need different concentration of DEXTROSE solution other than the one which are commonly available in the market such as D 25,D10 , D5etc.we can easily make these solutions very easily using D25(twenty five percent DEXTROSE) and ISOLYTE P
For this calculations ,there is a simple equation.The equation is,
5X-25=amount of D25 to be added to make 100 ml [here X is the concentration needed]
Example,if we want to make D12. 5 percent solution using D25 and ISOLYTE P .
here X=12.5,
so the equation 5X-25=amount of D25.
becomes
5(12.5)-25=37.5 ml D25
that is, to make 100 ml of 12.5percent solution,
we have to add 37.5ml of D25 and 62.5ml of ISOLYTE P .If it is for 50 ml D12.5solution, then add,
37.5/2=18.75ml D25 and 31.25ml ISOLYTE P
Similarly, If we want D20solution,Then X=20
then 5X-25=amount Of D25,becomes
5(20)-25=75ml D25
that is,
to make 100ml of 20 percent DEXTROSE
solution,we have to add 75ml. of D25 and 25 ml of ISOLYTE P.

Take this very simple formula of mine, if you want to make any concentration from any two available concentrations you need to take the difference between the desired concentration and the available low concentration and divided by the difference between the two available concentration, then multiply the number you get by 500 this will give you how much fluid you need to exchange to make the desired concentration in a 500 ML bag or you can multiply by 100, 200, depend on how much fluids you need.
ReplyDeleteExample: to make 12.5% dextrose from mixing 5% dextrose and 50% dextrose by using our formula above
(12.5 - 5)/(50-5)=
7.5/45= 0.166666
multiply the result by 500
0.166666*500=83.3 ml
so the amount you need to exchange is exactly 83.3 mls ie if you want to make 12.5 dextrose out of 5% dex bag you need to take 83.3 mls out and replace it by 83.3 mls of 50% dextrose.
another example, how to make 3% NaCl saline out of 0.9% saline and 10% NaCl
again it is deadly easy !!!!
(desired concentration-available concentration) all divided by (high concentration-available concentration)
(3-0.9)/(10-0.9) =2.1/9.1=0.2307
0.2307*500=115.3 ml
ie from a bag of 500 of normal saline you need to take out 115 mls and replace it by 115 mls of 10% NaCl to make 3% hypertonic saline
